Summary:
Responsible for planning, providing, arranging, and monitoring services to consumersparticipating in Comprehensive Community Services (CCS), Community Support Program(CSP), Targeted Case Management (TCM) under the close supervision of a Health andHuman Services Supervisor.
Essential Duties Performs clinical assessments including initial intake and yearly reviews.
Develops, directsand implements treatment plans, interventions, and crisis response plans using a trauma-informed and person-centered approach.
Ensures that the member receives culturally,linguistically, and stage-appropriate services in a timely manner.
Provides information,outreach, and assistance to adults and their families.
Refers consumer to contractedservices and monitors to ensure the consumer can achieve the highest level of independentfunctioning.
Provides individual and family psychoeducation and individual skill development incommunity-based and home settings.
Encourages natural support investment andparticipation in recovery concepts and processes.
Interfaces with healthcare providers andother social service agencies to coordinate services.
Functions as part of treatment teams/multi-disciplinary teams to ensure continuity of care.
Communicates effectively through verbal, non-verbal, and written communication.
Continually seeks to improve skills and demonstrates a high level of self-direction.
Maintain accurate and timely documentation in the form of assessments, treatment plans,progress notes, etc.
as required by agency policy and State statute requirements includingthe authorizing and distribution of funds when warranted.
Attends and participates inagency staffing and in-services.
Participates in one on one, side by side, and group supervision.
Other duties as assigned,including serving in a coverage role for other department staff.
Qualifications A minimum of a bachelor's degree in behavioral science and 1,000 hours of supervised post-degree clinical experience serving individuals with severe and persistent mental illness, or abachelor's degree in a field other than behavioral science with 2,000 hours of supervisedpost degree clinical experience serving persons with severe and persistent mental illnesspreferred.
Knowledge and understanding of federal laws, State statutes, and administrative ruleswhich regulate the services to be provided.
Knowledge and understanding of the programs, funds and services available.
Knowledge of and sensitivity to social problems, cultural diversity, human growth anddevelopment, and individual and family functioning.
Ability to coordinate activities and functions that cross unit and departmental lines andinvolve other county and community agencies.
Ability to establish and maintain effective working relationships with coworkers, superiors,community partners, and the public at large.
Ability to relate to people and to communicate effectively in both written and oral formats.
Ability to maintain confidentiality.
Ability to exercise sound judgment in confrontational and stressful situations.
Ability to follow instructions, to complete tasks, and to work closely with the supervisor andothers.
Must have unlimited access to a car, a valid driver's license, automobile insurance, and theability to efficiently travel in and out of county, as needed.
Working RelationshipsFrequent contact with supervisor, co-workers, consumers, State and County personnel,other authorities, community partners, and members of the public.
May occasionally deal with aggressive or violent individuals.

Work Environment & Physical Requirements Work is performed in a variety of home, community based and office settings and involvestravel.
Ability to lift up to 20 pounds and a combination of sitting and standing postures maybe required.
Ability to minimally work 8 hours per day and 40 hours per week is required.
Must be able to operate and be conversant with office equipment including phone,computer, facsimile, calculator, printer and photocopier.
Familiarity and capacity to workwith various software applications and keyboarding proficiency are essential.
