20 an hour 9 days ago 9 days ago 9 days ago Purpose and Summary Under the supervision of the ADRC Supervisor, provides information and assistance, connecting older adults and adults with disabilities and caregivers to programs and services, including public and privately funded options.
Provides assessment and long-term care options counseling services to older adults and adults with disabilities in Sheboygan County.
Offers Options Counseling built on the strengths of individuals, families, and the community.
Works collaboratively with internal and external services in order to advocate for consumer needs to assure services are delivered.
Essential Duties To provide the general public, particularly adults who are elderly or disabled, with information and assistance to a wide range of community resources; help inform and educate people about their options; assist in connecting them to programs, services and public benefits; and provide intake/on-call, including referrals to Adult Protective Services, eligibility determination and enrollment into Wisconsin long-term care benefit programs.
Administers the Long-Term Care Functional Screen to determine functional eligibility; provides information about publicly funded long-term care and assists consumers throughout the process.
Completes and maintains appropriate and timely documentation on consumers in the consumer records and databases; completes and maintains required agency documentation for 100% Time Reporting purposes.
Provides consumer-driven prompt, accurate, and friendly customer service responses to requests for information about an array of topics and for access to long term care resources; searches through the Information and Assistance resource database, as well as other written and computer-based information resources to identify, evaluate, and suggest potential programs and services; collects consumer information and assesses client/family needs in order to provide the information and assistance with a trauma informed care approach.
Performs other duties as assigned.

Qualifications A Bachelor's Degree from an accredited college or university in a human service related field or registered nursing field, and an understanding of the aging and disability population.
Work Environment & Physical Requirements Functional Screen Certification within 3 months of hire.
Lifting 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.
Maintain valid driver's license, access to reliable personal transportation and functional screen certification while employed in this position.
